How do you sign-up for MyChart

As soon as you schedule your first UI Health Care appointment, you’re eligible for a MyChart account. You can sign up through the MyChart website on your computer, or you can download our mobile app on your iPhone, iPad, or Android device.

No matter which device you use, you can sign up for MyChart using one of two options. Choose the one that fits your situation:

You are a new patient

Before you set up your MyChart account, we'll ask you for personal information to confirm your identity. Once your identity has been confirmed:

  1. On the sign up form, enter all required fields: name, address, date of birth, email address, and phone number.
  2. Tap or click the box next to “I’m not a robot.” The box should change to a green checkmark. You can tap or click the “Next” button. Or you may be asked to pass a simple test in which you identify objects in images. After you’ve done that successfully, the box will change to a green checkmark and you can tap or click the “Next” button.
  3. You’ll be asked a series of questions about yourself. This step helps us make sure that it’s really you creating a MyChart account.
  4. Once you’ve been verified, choose your username and password.
  5. Explore your new MyChart account!

You’ve had an appointment with a UI Health Care provider, and you have your after-visit summary

You should have received an after-visit summary as part of your appointment check-out. If you have an after-visit summary:

  1. On the activation form, enter your activation code, your medical record number, birthdate, andZIP code.
  2. Tap or click the “Next” button.
  3. Choose your username and password.
  4. Explore your new MyChart account!

What are the downsides of MyChart? ›

What is the exclamation point next to test results in MyChart? ›

3) Test results with an exclamation next to them indicate that a test result is outside of reference range. A test marked as outside of reference range is not always a reason for concern. Please discuss with your provider.

Why is the MyChart app not working? ›

If you are having issues with the MyChart app, please contact the MyChart Support line at 1-833-212-1586, Monday through Friday, 8am-6pm. Test Hardware - You have the ability to test your hardware prior to your scheduled video visit from your mobile device or through web browser.

Can you ask your doctor questions on MyChart? ›

Use MyChart Message Center to have conversations with your doctor or other clinic staff, ask non-emergency medical questions, send test results to your provider from another provider or network, and even upload your official COVID-19 vaccination record.

What is the difference between MyChart and Epic? ›

Note: MyChart is an Epic branded name. Your organization may use a different name for their patient portal. Access the application on your personal device. Carefully review the app's terms and conditions.

What does an H flag mean on MyChart? ›

If your blood counts fall outside of the normal range, which is shown in the “Reference interval” column, their values will be reported in the “Flag” column with an 'L' for low and an 'H' for high. The example CBC below shows that white blood cells, red blood cells, and platelets are all low. CBC WITH DIFFERENTIAL.

Do patients have a right to know their test results? ›

It depends upon what you have been diagnosed with. If you have been diagnosed with minor ailments the doctor will not call right away.

Why are my test results not showing up on MyChart? ›

Some test results are not released to your MyChart account because of their sensitive nature. Your doctor determines which types of test results are able to be accessed through MyChart and which might require an in person conversation.
